***IMPORTANT INFORMATION TO HELP YOU PASS YOUR DOT EXAM***
Bring COMPLETED Form MCSA-5875 (found below) to your appointment and arrive 10 minutes early. If you do not have access to a printer, you may fill out the form in our office, but you must come 15 minutes early to do this.
Drink some fluids prior to your appointment for the urinalysis. Bring your glasses/contacts for the eye exam. List your medications with the name and dosage on your MCSA-5875 form or attach a list.
If you have SLEEP APNEA, you must provide a ONE YEAR CPAP usage report. This can be obtained by calling the phone number on your Cpap machine. You may fax the report to us at 724-353-8610.
If you have DIABETES, you must provide a copy of your most recent A1C bloodwork report. You may fax to us at 724-353-8610.
If you have ANY ISSUE AT ALL WITH YOUR HEART OR EVER HAVE IN THE PAST, you must provide a recent cardiology report. You may fax to us at 724-353-8610.
Medical marijuana cards are automatic disqualifiers. The Federal government considers marijuana an illegal substance and you cannot drive a commercial vehicle and have a Medical marijuana Card.
If you need to change this appt, we require AT LEAST 24 hours notice. If you no-show for your appt, you will be charged a fee.
